  1. Claim Form
    a form used by a claimant to begin court proceedings
    Example He issued the Claim Form on Friday and served it on the defendant on Monday.
    (Note Since the introduction of the Civil Procedure Rules, this term has replaced writ of summons.)
  2. a form used to make an insurance claim
    Example He filled in the claim form and sent it to the insurance company.
COMMENT The Claim Form must be served on the defendant within four months of being issued. If a Claim Form has been issued but not served, the defendant can ask for it to be served, and if the claimant does not do so, the claim may be dismissed.
